This Notice must be mailed to all beneficiaries and heirs at law within 30 days of fiduciary qualification.
After sending Notice to Beneficiaries, this Affidavit of Notice is to be filed directly with the Clerk of the Circuit Court.
To be filed with FINAL accounting for Decedent's Estate.
The Commissioner has responsibility to protect creditors of the Estate.
The Commissioner cannot approve a final Account unless all claims against the Estate have been resolved.
Similarly, no fiduciary can file a Statement in Lieu of Account while a claim is outstanding.
A claim may be filed with the Commissioner’s office by completing the claim form below and mailing the completed form, along with the required $95.00 filing fee, to the Commissioner’s office at the address listed on the form.
A copy of the form must also be sent to the personal representative of the Estate.
Please note that claims may NOT be lodged against Conservatorships or Trusts.

For any upward departure from the Guideline Fees or compensation specified in the Will, all residuary beneficiaries must sign the completed attached form.
